Abstract
Objective To study the clinical efficacy of different dosage of valsartan combined with amlodipine in the treatment of elderly hypertension. Methods A total of 260 elderly hypertensive patients in our hospital were selected from 2010 to 2013,then they were randomly divided into observation group and control group 130 cases in each group. Patients in control group were treated with valsartan combined with amlodipine,while patients in observation group were treated with double dosage of valsartan combined with amlodipine. The changes of blood pressure and renal function of the two groups before and after treatment were observed,and the relationship between the index of blood pressure and renal function were analyzed. Results Blood pressure of the two groups after treatment were improved than before treatment,the difference were statistically significant( P 0. 05),while there was no statistically significant differences of 24 h average systolic blood pressure( 24 h SBP) and 24 h average diastolic blood pressure( 24 h DBP) between the two groups( P 0. 05). 24 h average systolic blood pressure variability( 24 h SBPV) and 24 h average diastolic blood pressure variability( 24 h DBPV) of the two groups after treatment were improved than before treatment,the differences were statistically significant( P 0. 05),and those of observation group were better than control group,the differences were statistically significant( P 0. 05). Renal function of the two groups after treatment were improved than before treatment,serum creatinine( Scr),blood urea nitrogen( BUN),endogenous creatinine clearance rate( Ccr) of observation group were better than those of control group,the differences were statistically significant( P 0.05). In the two groups,24 h SBPV,24 h DBPV after treatment was positively correlated with Scr,BUN( r = 0. 21,0. 26,r = 0. 19,0. 23,P 0. 05),respectively was negatively correlated with Ccr( r =- 0. 27,- 0. 25,P 0. 05),respectively. Conclusion Double dosage of valsartan combined with amlodipine can significantly improve the circadian blood pressure variability and renal function in elderly hypertensive patients.
