Abstract
OBJECTIVE To evaluate the effectiveness of hypertension community management and improve the control of hypertension.METHODS Literature review and Delphi method were used to formulate intervention measures.Evaluate the implementation of intervention project through field investigation.RESULTS Blood pressure control rate,weight loss rate and waist down scale,hypertension management rate all have achieved outstanding.However,there were no significant differences at the knowledge of hypertension,diet,alcohol consumption and smoking.CONCLUSION The intervention on hypertension is still a long-term process.
