The Application of “4CH8”Model Based on Health Management in Community Hypertension Management

Objective To evaluate the effectiveness of4CH8community hypertension management model,and explore the effective new model in community hypertension management. Methods 2 community service centers were selected randomly in Shanghai,from which we investigated 300 hypertension patients by random sampling separately before and after the intervention. Pairing Chi-square test and T test were also used to compare the indexes before and after the intervention. Results After the intervention,the blood pressure,blood lipid,complication rate and BMI of intervention patients were decreased,whose disease awareness was improved. Compared with control group,the differences of intervention between before and after were significant. Conclusion 4CH8Model had some clinical effect in community hypertension management,which can provide reference for building new chronic disease management model in community.
