Abstract

Objective To observe the clinical therapeutic effect and safety of carvedilol in the treatment of chronic heart failure. Methods 76 patients with chronic heart failure were randomly divided into treatment group and control group. 38 patients in control group were treated regularly (diuretics, angiotensin-converting enzyme inhibitors and digoxin), however, the other 38 patients in treatment group, on the therapy basis of control group , were administrated with carvedilol. The dose of carvedilol was changed from 3. 125mg twice a day to the target dose 25 mg twice a day or the maximum tolerant dosage by duplicating the former dose every two weeks, with a treatment course of 6 months for both groups. The changes of blood pressure (BP), heart rate (HR),left ventricular diastolic diameter (LVDd) ,left ventricular systolic diameter (LVSd) and left ventricular ejection fraction (LVEF) were observed before and after the treatment. Results The effective rate in treatment group (94.74%) was significandy higher than that (73.68% ) of control group ( P 0. 01 ). After treatment the BP, HR and parameters of cardiac function in both groups were improved obviously, as compared with those before treatment ( P 0. 05 or P 0. 01), and the improvement in treatment group was more obvious than that of control group ( P 0.05 or P 0.01). Conclusion The target dose of carvedilol is safe and effective in the treatment of chronic heart failure.
