Goal and Effectiveness of Combined Community Health Education Program for Elder Patients with Diabetes

Abstract

Objectives To observe the effect of the Joint target health education for elder diabetic patients in community.Methods 80cases of elder diabetic patients from the Third People's Hospital of Yichang City were divided into intervention group and the conventional group by community identity,40cases in each group.Both groups were followed-up.The intervention group was given joint health education of hospital,community and families.Conventional group underwent routine community health education.Glucose,lipids and other blood biochemical indicators and changes in the quality of life of all patients were recorded.Results Biochemical indicators in blood were effectively controlled.SF-36scale evaluation and self-assessment of the quality of life of the intervention group were significantly higher than those in the conventional group.Conclusions Implementation of the joint target health education in community for elderly patient with diabetes could help control blood sugar and improve the quality of life in patients
