Abstract
Objective To explore the intervention of community care for diabetes patients living habits and the effect of the treatment. Methods 164 cases of diabetes were randomly divided into two groups of patients and control groups,the experimental group for community care guidance intervention,and for each individual patients to guide the control group did not conduct the diabetes health education. Six months after the diabetes patients to monitor blood glucose and changes in living habits. Results The patients with diabetes living habits changed significantly higher(P0.05),good blood sugar control rate in the control group.There was a significant difference: Conclusion Community care interventions can improve patient self-management capacity and efficacy,improving the quality of life.
