Abstract

Objective To study the early diagnostic value of multi-slice spiral CT perfusion imaging (CTP)I in acute cerebral ischemia. Methods Conventional and perfusion CT scanning were performed in fifty patients suspected acute ischemia with symptom onset within one and 12 hours.CT and clinical follow up were obtained in all patients in 3~7 days after the first time of CT examination. Results In the first conventional CT scanning,21 patients were detected lesions of infarction,29 patients had negative results, but in the 29 patients,24 patients were confirmed with infarction,3 patients were confirmed with lacunar infarction, and 2 patients were confirmed with transient ischemic attack(TIA) after CT reexamination in the followup. In CT perfusion images,48 patients had positive results and 2 patients had negative results in 1~12 h after symptom onset. The values of region of interest(RO)I in regional cerebral blood flow(rCBF),time to peak(TTP) and mean transit time(MTT) were changed obviously, there was a significant difference between the side of ischemic lesion and the opposite side, between the center of ischemic lesion and peripheral zone. Conclusion CTPI can diagnose acute cerebral ischemia in early stage and can identify the core of infraction. It is helpful for selecting therapy planning and treatment of thrombolysis for acute cerebral ischemic patients.
