Abstract
To investigate the effects of closed drainage by central venous catheter and intrapleural urokinase in the treatment of encapsulated tuberculous pleurisy(ETP).Methods:40 cases with ETP were randomly divided into two groups: treatment group(20 cases)and control group(20 cases).The patients in treatment group was intrapleurally injected with urokinase 100,000 IU plus 20ml of 0.9% N.S by central venous catheter,while the control group was injected with 20ml of 0.9% N.S. Results:The mean volume of drainage fluid in the treatment group was more.Pleura adhesion disappearance(PAD)was 15 cases and the control group was only 2 cases of PAD.2 months later,guided by B-ultrasonography,the mean thickness of pleura in treatment group was(2.5+0.8)mm but(9.8+1.6)mm in control group(P<0.01).The extent of chest collapse was also different, 5.3+1.6% in the treatmem group and 11.1+2.7% in the control group.There were significant differences in arterial blood gas analysis and lung function before and after trapleural urokinase therapy(P<0.01).Conclusion:Intraplearal urokinase injection treatment can effectively increase the drainage,decrease pleura adhesion and pleural thickening,and improve pulmonary function of patients.
