The analysis of operative efficacy for glaucoma complicated with cataract

Objective To evaluate the surgical selection and its effect for glaucoma complicated with cataract.Methods A retrospective study was performed on 47 patients(56 eyes) of glaucoma with cataract.12 eyes were open angle glaucoma and 44 eyes were angle-closure glaucoma.5 eyes with angle open more than 180 degree had phacoemulsification combined with intraocular lens(IOL) implantation,and the others underwent trabeculectomy combined with phacoemulsification,besides 2 eyes with IOL implantation in the same stage.Results The postoperative intraocular pressure(IOP) of 49 eyes were normal,and the rest of 7 eyes were in control after antiglaucoma medications or secondary operation.The postoperative vision improved except light perception of 3 eyes,the same as the preoperation.Moreover,54 eyes had good anterior chamber form,while anterior chamber forming surgery applied for the other 2 eyes because of the delayed flat anterior chamber.Conclucion The patients with angle-closure glaucoma which is first onset,angle open more than 180 degree and the aggravated papillary block because of the lens opacity,have an indication for phacoemulsification combined with IOL implantation.Phacoemulsification combining with trabeculectomy for glaucoma complicated with cataract can control IOP effectively and improve visual ability.It can avoide the secondary surgery and reduce the rate of shallow anterior chamber.
