Abstract
Objective To research the method and effect of sequential non-invasive following short-term invasive mechanica1 ventilation in chronic obstractive pulmonary disease(COPD) patients with severe respiratory failure.MethodsThirty six COPD patients with severe respiratory failure were studied.When pulmonary infection had been controlled,half of the patients(treatment group)were extubated and treated by non-invasive mechanical ventilation,the other half(control group)were kept treating by invasive mechanical ventilation and weaned by PSV mode.The duration of invasive mechanical ventilation and total invasive mechanical ventilation and hospitalization and care,the incidence of ventilator-associated pneumonia(VAP) and successive weaning and death in hospital,exhausting in hospital were compared between treatment group and control group.ResultsFor treatment group and contro1 group,the incidence of VAP was 0 VS 8 cases(P0.05);the tota1 duration of mechanica1 ventilation was(14.2±1.3)d VS(22.1±0.7)d(P0.05);the duration of hospitalization was(17.3±1.5)d VS(26.8±2.4)d(P0.01).ConclusionSequentia1 non-invasive following invasive mechanica1 ventilation at the time of pulmonary infection contro1(PIC)window is a valuable and effective project by decreasing the duration of mechanical ventilation and the morbdity of VAP,and the length of ICU and hospital stay,as well as the cost of therapy,also increasing the effect of therapy.
