Risk factors analysis of intracranial infections in patients with craniotomy

Abstract

OBJECTIVE To analyze risk factors of intracranial infections in patients with craniotomy,so as to figure out corresponding preventive measures to effectively reduce the incidence of intracranial infections and improve its prognosis.METHODS The clinical data of 674 cases of patients who received craniocerebral operation from Feb.2007 to Feb.2013 were retrospectively analyzed,as well as the difference of patients with intracranial infection at the following aspects:age,gender,GCS score,the way of injury,timing of surgery,whether postoperative drainage tube was set up,whether antibiotics was used preoperatively,surgery duration,operative sites and whether there was a leakage of cerebrospinal fluid and other factors.All the data were processed with SPSS13.0.RESULTS There were 125 cases of intracranial infection among the 674 cases,thus the infection rate was 18.5%.Univariate analysis showed that there was a significant difference between groups(P0.05)in surgery duration(≥4h),operating on the posterior fossa,cerebrospinal fluid leakage and open craniocerebral injury,of which the infection rate was respectively 29.3%,22.1%,24.0%and 27.6%.Multiple logistic regression analysis showed that operation time,operative sites,cerebrospinal fluid leakage and the ways of injury were the independent risk factors of intracranial infection after craniotomy(P0.05).CONCLUSIONS Surgical duration,operative sites,cerebrospinal fluid leakage and the way of injury are the risk factors for intracranial infection.Relevant preventive measures should be strengthened to reduce the incidence of intracranial infections.
