Abstract
ObjectiveTo investigate the variation of pathogenic bacteria and resistance of urinary pathogens in children with urinary tract infection(UTI)in our region during last eight years period.MethodsRetrospective analysis of isolat ed bacteria from children with UTIs was performed at our hospital during year 2005—2012.ResultsA total of 843uropathogens were isolated,dominant with Gram negative organisms(84.93%).The most common causative Gram-neg tive bacteria was Escherichia coli(E. coli)(62.75%),Enterococcus faecium(7.12%)was the leading cause for Gram positive organisms. Compared with the prevalence of bacterium spectrum in 2005—2008,prevalence of enterococcus faecium in 2009—2012 was significantly increased(2.43% vs 11.57%,P0.01).Among the Gram negative organisms,the resistance of Escherichia coli(E. coli) and Klebsiella to ampicillin was the highest. Imipenem appeared to be the most effective antibiotic. It was observed that resistance of E.coli to cephalosporins and Ampicillin/Sulbactam(P0.05),Proteus mirabilis to ciprofloxacin and nitrofurantoin(P0.01),enterococcus faecium to penicillin(P0.05),and the resistance of enterococcus faecal to gentamicin(P0.05)increased significantly.ConclusionsGram negative organisms are the most common causative agent in children with UTIs,but Gram positive organisms have increased. In re cent years,the resistance prevalence of urinary pathogens to commonly used antimicrobials was varied.For lower urinary tract infection,nitrofurantoin may be appropriate,and the third generations of cephalosporins could be used for non-complicated urinary tract infections. However,in refractory and severe cases or those with complicated factor,β-Lactamase inhibitors combined drugs or Imipenem are recommended. If Gram positive organisms infection are consid ered,Vaneomycin and Linezolid could be optional.
