Effect of different doses of preoperative finasteride on the bleeding during transurethral resection of prostate

Objective To evaluate the effect of different doses of finasteride administered before operation on the volume of bleeding during transurethral resection of prostate(TURP),and to study its mechanism.Methods During a randomized controlled trial,a total of 90 patients with benign prostatic hyperplasia who would receive TURP treatment were divided evenly into three groups.The 5mg group and the 10 mg group respectively took 5mg and 10 mg of Finasteride daily for two weeks before operation,and the control group didn't take Finasteride.The weight of removed prostatic tissue,the operative bleeding volume,MVD and VEGF expressions in the prostatic tissues of each group were measured and compared.Results The removed prostate weighed(22.3±6.2)g for the 5mg group,(22.5±6.5)g for the 10mg group and(23.2±5.3)g for the control group,with no significant differences(P0.05).Compared to the control group,the operating time,mean operative bleeding,MVD volume and mean positive cell count of VEGF of the two Finasteride groups were significantly reduced(P0.05),but there was no statistical difference between the 5mg group and the 10mg group(P0.05).Conclusions 5 mg Finasteride daily for two weeks before operation can effectively suppress the VEGF expression and angiogenesis in the prostate tissue,and therefore shorten the operating time and decrease the blood loss during TURP.
