Comprehensive community hypertension self-management support intervention and effect evaluation

Objective To study the Urumqi community of self-management support intervention and prevention and treatment of high blood pressure effect.Methods Court of Urumqi Lu-yuan Street community 258 cases of hypertensive patients to assess filing.Planned for community intervention and self-management support,and follow-up three years later,comparing patients before and after their health behavior and blood pressure control.Results 258 cases of hypertension in high-salt diet,smoking,lack of exercise and other adverse health behaviors were significantly improved compared with before intervention(P0.01).Treatment significantly increased(P0.01).Conclusion Community Health Center of the hypertensive population a comprehensive self-management support intervention and prevention and treatment of high blood pressure is one of effective methods.
