Abstract
Objective To investigate the effects of open lung strategy in mechanical ventilation treatment of moderate to severe neonatal respiratory distress syndrom (NRDS).Methods Fifty - six cases of neonates with NRDS were randomized into open lung ventilation group ( group Ⅰ ) and routine ventilation group (group Ⅱ ),each group of 28 patients.In addition to general treatment,both groups were treated with PC + SIMV ventilation model,lung recruitment maneuver in group Ⅰ and traditional ventilation in group Ⅱ.The arterial blood gas results were compared between the two groups before ventilation and 6,12,24,48 hours after ventilation,the complications and the curative effect were compared between two groups.Results The results of PaO2,SaO2,PaO2/FiO2 after ventilation increased remarkably,compared with those before ventilation in two groups.The results of PaO2,SaO2,PaO2/FiO2 of group Ⅰ after ventilation increased more significantly than group Ⅱ after ventilation in same time point( P <0.05) ; the incidences of VIPI decreased remarkably in group Ⅰ than that in group Ⅱ,the incidences of VIH/PVL in two groups were no significant change( P > 0.05 ),the curative effect of group Ⅰ was increased more obviously than group Ⅱ( P < 0.05 ).Conclusions Open lung ventilation can improve the oxygenation remarkably,decrease the incidences of VIPI and increase the curative effect of NRDS.
