Abstract
Objective To evaluate the effect of the collaboration care model on self care and quality of life in patients with hemodialysis. Methods A total of 80 patients with hemodialysis were randomly divided into observation group and control group (40 cases in each group). The patients of control group received routine care, while the patients of control group underwent the collaboration care model based on routine care. Six months after the intervention, the self care scale and lifestyle scale were used to compare self-care ability and health promoting lifestyle before and after interventions. Results After intervention in the observation group, the total scores of knowledge level of hemodialysis, self care, compliance and nursing ability were higher than those of the control group (P<0.05); the physiological function, social relationship, sports and quality of life presented the same trend between two groups (P<0.05). Conclusions The collaboration care model can significantly improve the patients′ self care and quality of life. Key words: Hemodialysis; Self care; Quality of life; Collaborative care model
