Influence of collaboration care model on self-care ability of hemodialysis patients

Abstract
Objective
To explore the influence of collaboration care model on self-care ability of hemodialysis patients.
Methods
A total of 70 cases of patients with end-stage renal disease were selected from June 2012 to May 2013 in our hospital, using collaborative care intervention. Scores of self-care ability, self-management ability and general quality of life were compared before and after the implementation.
Results
After collaboration care model, the scores of self-care ability, self-management ability were (115.14±11.25) and (30.28±3.26), which were significantly higher than those before (t=4.254, 3.215, respectively; P<0.01). Except two dimensions of physical functioning and physical role, the scores of the rest six dimensions had statistically significant differences (P<0.01).
Conclusions
Implementation of collaboration care model in patients with hemodialysis can improve the abilities of self-care, self management and the quality of life.
