The effect of preoperative short-term usage of Large doses finasteride on reducing hemorrhage during transurethral resection of prostate

Abstract

Objectives To study the effect of finasteride preoperatively taken on reducing bleeding in transurethral resection of prostate. Methods 104 patients were equally divided into 2 groups, 52 of them had taken finasteride and the other 52 did not. The amount of bleeding during the resection in both groups was compared. CD34、bcl-2 、PCNA and VEGF expressions were measured by ISH SP-method in the resected tissues in both groups. Results The total amount of bleeding per minute and bleeding per gram of the tissues were lower than those in the control group, There was a singnificant difference(p <0.05). The microvascular density as studied by immunobistochemistry was much less in the fenasteride group, the mean MVD volume 60 ±2.4 and 123 ±3.9 (p < 0. 05). The expressions of bcl-2、 PCNA、 VEGF were also different between the two groups(p <0.01). Conclusions Finasteride might lessen prostate bleeding associated with prostatectomy. It s mechanism is that finasteride can inhibit the angiogenesis of prostatic tissues. Key words: Prostatectomy;  Urethra;  Electrosurgery;  Androstenes
