Partial splenic embolization in treatment of hypersplenism by hepatic cirrhosis and portal hypertension pressure of hepatic cirrhosis

Abstract
Objective To investigate the treatment effect of partial splenic embolization(PSE) on hypersplenism by hepatic cirrhosis and portal hypertension.Methods Antibiotic was used regularly in preparation for intestines before and after operation.The 28 cases with hypersplenism and hepatic cirrhosis underwent PSE,18 out of which through super-selective catheter into the splenic lower pole artery,10 through catheter into the door of spleen by means of low pressure flow control way.The forms of embolism were PVA granule with 300-500um,gelatin sponge strips or and granule.And the changes of white blood cell and blood platelet were observed in periphery blood before and after operation and reactions after operation.Results The spleens were examined through CT or and B ultrasonic wave detector after operation,the embolization areas was controlled as 40%~80% of the spleen.The infarction part of the spleen showed forms of honeycomb seven days after operation,and shrank a month after operation at varying degrees.White blood cells and platelets in periphery blood after operation were higher than before,and were in the normal or near normal level.At varying degrees,ascites decreased.Incidence of the splenic embolization syndrome was 100%.Conclusion PSE not only retains immunity function of spleen,but also decreases portal vein maximum and relieves hypersplenism effectively,is an effective treatment of hypersplenism.
