OBJECTIVE:To investigate the protective effect of ulinastatin on inflammatory reaction in patients who underwent esophagectomy.METHODS:Eighty esophageal carcinoma patients were randomly divided into two groups(n=40 each),a ulinastatin group and control group.The ulinastatin group recevied 2×105 units of ulinastatin continuosly for 3 days before the operation and 7 days after the operation.Serum TNF-α and IL-1β concentrations were detected at 0,6,24 and 72 h before and after the operation by the ELISA method.Oxygen indexes(OI) in aterial blood gas were detected at 0,6,24 and 72 h before and after the operation in both groups.RESULTS:The serum levels of TNF-α and IL-1β,and OI had no significant difference between the two groups before the operation,tTNF-α=1.357,P0.05,tIL-β=1.41,P0.05,and had a significant difference between the two groups after the operation,0 h tTNF-α=3.616,P0.01,tIL-β=6.46,P0.01;6 h tTNF-α=2.559,P0.05,tIL-β=12.26,P0.01;24 h tTNF-α=3.731,P0.01,tIL-β=0.23,P0.05;72 h after operation,tTNF-α=1.943,P0.05,tIL-β=1.14,P0.05.Ulinastatin could decrease the TNF-α and IL-1β significantly.OI in ulinastatin group was increased significantly than that in control group(P0.01).CONCLUSION:Ulinastatin can reduce the serum levels of TNF-α and IL-1β significantly and has a certain protective effect on lung inflammatory reaction.
