Distribution and drug resistance of pathogens from bile culture

Abstract

OBJECTIVE To explore the distribution and drug resistance of pathogens from bile culture so as to provide laboratory basis for targeted treatment of biliary tract infections.METHODS The bile specimens were collected from the patients with biliary tract infections through aseptic method,then the isolation,culture,and identification of the pathogens were performed by using routine clinical microorganism operation procedures,the drug susceptibility testing was performed with the use of disc diffusion method recommended by the WHO;the inhibition zones were measured,and the result of the drug susceptibility testing was determined by referring to the breakpoints of CLSI;the data were processed through WHONET5.5software.RESULTS Totally 227strains of pathogens have been isolated from the 409retained bile specimens with the isolation rate of 55.5%,including 151(66.5%)strains of gram-negative bacteria,49(21.6%)strains of gram-positive bacteria,and 27(11.9%) strains of fungi;the Escherichia coli,Enterococcus faecalis,and Klebsiella pneumoniae ranked the top 3species, accounting for 36.1%,14.5%,and 11.9%,respectively.The isolated pathogens were highly resistant to the commonly used antibiotics;the gram-negative bacteria were highly susceptible to the carbapenems antibiotics;the drug resistance rates of the E.coli,E.faecalis,and K.pneumoniae to imipenem and meropenem were 0;the drug resistance rates of the Enterococcus sppto vancomycin and teicoplanin were 0.Totally 55isolates of ESBLs-producing E.coli and K.pneumoniae have been detected,and the detection rate of the ESBLs-producing bacteria was 50.5%.CONCLUSIONThe intestinal flora predominate in the pathogenic bacteria causing the biliary tract infections,and the drug resistance shows an upward trend;it is necessary for the hospital to execute the management of clinical application of antibiotics issued by the Ministry of Health and enhance the idea of etiological test of the clinicians so as to improve the level of clinical treatment of biliary tract infections.
