Endoscopic nasal sinus surgery for type III sinusitis and nasal polyps

Abstract

【Objective】To study the operation procedure and effects of endoscopic nasal sinus surgery for Type Ⅲ sinusitis and nasal polyps. 【Methods】95 cases (190 sides) with Type Ⅲ sinusitis and nasal polyps were performed with endoscopic nasal sinus surgery. Rates of clinical effectiveness and complications were analyzed. 【Results】The long term follow up was performed in more than 6 months after operation, the results showed that 128 sides were cured (67.4%), 50 sides achieved symptomatic relief (26.3%) and 12 sides remained failure (6.3%). Operative complication included lamina papyracea lesion (6 sides, 3.2%), adhesion or occlusion of nasal cavity (21 sides, 11.1%) and hemorrhage more than 400 mL (3 sides, 1.6%).【Conclusions】Type Ⅲ sinusitis and nasal polyps were reactive inflammatory disease which had deletion or alteration of normal anatomic structure, and these would lead to higher risk on the operation, worse therapeutic effects and increase complication incidence rate. So careful search and ascertainment of landmarks of sinus anatomy, careful disposal of middle nasal turbinate and uncinate process and adequate preoperative and postoperative glucocorticoids are important factors in reducing inflammation and promoting healing rate for endoscopic nasal sinus surgery on type Ⅲ sinusitis and nasal polyps.
