Analysis of combined spinal-epidural anesthesia with Bupivacaine with low-dose and light weight fluid in the application of elderly lower limb fracture surgery

Objective:To discuss the feasibility of combined spinal-epidural anesthesia with 0.5% Bupivacaine with 0.8-1.0 ml, light weight and low-dose liquid in the elderly lower limb fracture surgery.Methods:60 patients aged 65-92 years with femoral neck fracture surgery, accepted the lateral position, single space combined spinal-epidural anesthesia, selected L3-4 or L2-3 for the puncture points, with 25G lumbar puncture needle, according to the patient's age and general situation, injected 0.5% Bupivacaine 0.8-1.0 ml in spinal anesthesia.Through the epidural space inserted the cephalic epidural catheter 3 cm, set up perfect hemodynamic monitoring.Results:The spinal anesthesia and epidural catheter were successful in 56 cases, the success rate was 93%;after 3-5 min when spinal anesthesia injection succeeded, that sense of lower limb pain dissappeared, the anesthetic effect was satisfactory, it could meet the required surgery.Among 30 min since subarachnoid administration, every 5 min, a total of five time points, the SBP, DBP, HR compared with the basic value of preoperative comparison, there was no significant difference(P0.05).Conclusion:Combined spinal block anesthesia with 0.5% Bupivacaine of 0.8-1.0 ml, light weight and low-dose fluid can fully meet the needs of the elderly lower limb surgery, and it is safe, reliable, and has no significant side effects.
