ANALYSIS OF BI-LEVEL POSITIVE AIRWAY PRESSURE TREATMENT FOR AECOPD WITH TYPE II RESPIRATORY FAILURE

Abstract

[Objective]To investigate the clinical efficacy of bi-level positive airway pressure treatment for chronic obstructive pulmonary disease in acute stage(AECOPD) with type Ⅱ respiratory failure.[Methods]Selected 56 AECOPD with type Ⅱ respiratory failure patients hospitalized from January 2009-June 2010,all were randomly divided into group and control group,each of 28 cases.Bi-level positive airway pressure treatment was given to observation group,control group received conventional therapy,both groups were observed before treatment and after treatment,blood gas changes,and the results were analyzed.[Results]After treatment,effective rate in observation group was 89.29%,significantly higher than the control group 71.43%(P﹤0.05) .There was a significant difference in effective rate between two groups(P﹤0.05) .Before treatment,PaO2,PaCO2 levels were equivalent between the two groups(P﹥0.05) .After treatment,PaO2,PaCO2 in both groups were improved,and which were better in the observation group than the control group(P﹤0.05) .Breathing rate and heart rate in observation group were lower than the control group(P﹤0.05) .Systolic and diastolic blood pressures in two groups were not significantly different.[Conclusion]The bi-level non-invasive mechanical ventilation in chronic obstructive pulmonary disease with type Ⅱ respiratory failure is effective,and can improve ventilatory function in patients,which is worthy for the clinical application.
