Status of Chronic Disease Management

With the increasing incidence of chronic non-communicable diseases,chronic disease management has become the focus of global attention.Chronic disease management aims to provide comprehensive,continuous and proactive management of patients with cardiovascular disease,cancer,diabetes,chronic lung disease and so on by chronic disease professional doctors,pharmacists and nurses.In abroad,a variety of chronic disease management models have been designed or improved based on the different diseases,and a series of related policies have been issued.In our country,chronic diseases are managed mainly in communities,researchers are exploring the assessment of current situation,theoretical framework,appropriate technology and management platform of chronic disease management.Related specifications have been issued,but a generally accepted scientific and effective model has not yet formed,and there are many problems in existed models.This article reviews the chronic disease management models home and abroad.
