Abstract
Non-communicable chronic disease has become the major health related problem of the community inhabitants of our country.The management of patients can not be ignored in the task for the prevention and cure of chronic disease.Basing on generous domestic and overseas literatures,this article has discussed the international actuality of the application of disease management and achievements.Then discusses the models of the disease management in order to provide a good reference its for non-communicable chronic disease management.
