Research Practice and Problem Discussion of Self-management of Chronic Diseases

Self-management of chronic diseases is an effective practice to manage and control chronic diseases.It is more and more concerned in China although it has not been widely accepted and applied.This paper comprehensively reviewed the latest researches and practices of self-management on chronic diseases.Through introducing current epidemiology of chronic diseases in China,the relevant concepts and contents of chronic diseases self-management,it aimed to explore the main problems and solutions of self-management on chronic diseases,hoping to provide practical evidence and strategies for prevention and control of chronic diseases in China.
