Abstract
Objective To make a correct diagnosis of gastrointestinal mesenchymal tumor by observing the endoscopic and pathological features from the 53 clinical cases. Methods The data about the endoscopy,pathology and immunohistochemical index including CD117,CD34,SMA and Desmin were collected.The clinical and endoscopic characteristics,pathological and immunohistochemical features were analyzed. Results There were different imaging features between gastrointestinal stromal tumors and leiomyoma.Most of the stromal tumors were located in the stomach and leiomyoma in the esophagus,the ratios were 64.71% and 68.42%,respectively.CD117 and CD34 were diffusely and strongly expressed in stromal tumors,but SMA and Desmin were focal expressed correspondingly with lower rates.Stromal tumors located in small intestine showed higher rate in SMA and lower rate in CD34.On the contrary,there were diffuse and strong expressions of SMA and Desmin in gastrointestinal leiomyoma,and the positive rates of CD117 and CD34 in leiomyoma were significantly lower than those in stromal tumors.According to correlation analysis,the negative expression of Desmin contributed to the diagnosis of stromal tumors and the negative expressions of CD117 and CD34 contributed to the diagnosis of leiomyoma. Conclusion The united immunohistochemical index detection of CD117,CD34,SMA and Desmin combined with endoscopic ultrasonography could enhance the accuracy in differentiation diagnosis between gastrointestinal stromal tumors and leiomyoma.
