Living quality research related to patient′s health after bricker operation and urinary diversion of orthotopic ileal neobladder

Abstract

Objective To study and explore the clinical effect of bricker operation and urinary diversion of orthotopic ileal neobladder in radical cystectomy and urinary diversion and analyze the influence of living quality relating to patient′s health by the two methods. Methods From June 2012 to December 2014,98 patients undergone radical cystectomy and urinary diversion in our hospital were selected as research objects.According to different urinary diversion modes,they were evenly divided in two groups and provided with orthotopic ileal neobladder and bricker operation.The clinical effectiveness in the two groups was compared and analyzed.Three-month follow up visits at least were needed targeting on comparing the influences of living quality relating to health by different therapeutic methods. Results There was no significant difference in operation time,intraoperative amount of bleeding,or hospital stay in both groups(P 0.05).Early-stage incidence of complications in the ileal bladder group was 44.9%,while in the orthotopic ileal neobladder group,the incidence was 49.0%,which didn′t display obvious difference in inter-group(P0.05).There were18 cases failed to follow up including 8 dead cases due to neoplasm recurrence and metastasis,6 dead cases due to non-tumor,and the rest 4 cases losing follow up for other reasons.During follow-up visits,76 FACT-BL questionnaires were delivered,and 62 effective ones were acquired accounting for 79.5%.There were 32 effective questionnaires and 30 ones in the ileal bladder group and orthotopic ileal neobladder group respectively.Scores of physical health status,family and social condition,and bladder cancer in the orthotopic ileal neobladder group were superior to those in the ileal bladder group with statistical differences(P0.05).Difference of total score of FACT-BL also displayed statistical significance(P0.05). Conclusion For bladder cancer,radical rectomy combining with bricker operation or urinary diversion of orthotopic ileal neobladder both can effectively solve the clinical problems.Urinary diversion of orthotopic ileal neobladder has some superiority in improving patient′s living quality.The appropriate method should base on individualized condition and personal will.
