Abstract
Objective: To study prospectively the curative effect of treating patients with type 2 diabetes nephropathy combining with valsartan and benazepril. Methods: One hundred of thirty-five patients with type 2 diabetes in the mass albuminuria rang were randomly divided into three groups. 65 patients were treated with benazepril 10 mg / d combined with valsartan 80 mg / d. 37 patients and 33 patients were given benazepril 10 mg / d and valsartan 80 mg / d respectively. Before and after one year therapy,the mean arterial pressure,urinary albumin excretion rate,serum creatinine urine IgM excretion( IgM),blood urea nitrogen,glomerular filtration rate,fasting blood glucose,two-hour postcibum blood glucose,hemoglobinA1c,serum potassium and blood lipid profile were determined. Results: Before and after therapy,there were no significant differences in fasting blood glucose,two-hour postcibum blood glucose,hemoglobinA1c,serum creatinine,blood urea nitrogen,mean arterial pressure,blood lipid profile and serum potassium among the three groups. Urinary albumin excretion rate,serum creatinine and urine IgM in all the groups showed significantly decreases after treatments( P 0. 05),and glomerular filtration rate increased significantly in all the groups( P 0. 05). There were significant differences in the group combined valsartan with benazepril( P 0. 01). But there were no significant differences between valsartan and benazepril groups( P 0. 05). Conclusion: It is an effective way to treat patients with type 2 diabetes in the mass albuminuria rang combined with valsartan plus benazepril,it can decrease in urinary IgM and albumin excretion,and it can increase in urinary IgM and albumin excretion,regardless of the influence on the blood pressure.
