Abstract
To investigate the expression of ICAM-1 in the intestinal mucosa and its relation with the mucous injury,bacterial translocation and endotoxemia in severe acute pancreatitis. SAP model was developed by intradutal injection of 5% sodium taurocholate in to pancreatic duct of healthy SD rats. The serum levels of AMY, LIP, LPS and the expression of ICMA-1 in the intestinal mucosa were examined respectively at 2, 6, 12 and 18 hours after operation for both the SAP group and the sham operation group. At the same time, the bacterial culture of samples from pancreas, liver, lung, spleen and mesentery were made respectively and changes in pathology were observed by using light and scanning electron microscopy in order to find the internal relationship between them. Compared with sham operation groups, the serum levels of LIP, LPS, AMY and the expression of ICAM-1 in the intestinal mucosa increased significantly in SAP groups(P0.01) and achieved the peak value within 12 hours and maintained at this levels until 18 hour after SAP operation. The bacterial culture of pancreas, liver, lung, spleen and mesentery all were positive in SAP groups and also there was positive correlation between the expression of ICAM-1 and the serum level of LPS. The intestinal mucous injury was remarkable after induction of SAP, especially at SAP12h group, including epithelial necrosis with desquamation. The high expression of ICAM-1 in intestinal mucosa probably led to epithelial necrosis and intestinal barrier dysfunction. As a result of intestinal barrier dysfunction, enteric bacterial translocation and endotoxemia might be proceeding.
