Clinical study on Carvedilol for cardic function neurohormonal activation and vascular endothelial function in patients with congestive heart failure

Abstract

Objective To assess systematically the effects of Carvedilol on the neurohormonal activation,vascular endothelial function and its clinical effect in patients with congestive heart failure. Methods Sixty patients with congestive heart failure were randomly divided into Carvedilol group and control group.The patients in control group received the routine treatment,including digitalis,diuretic and so on,except for Carvedilol.The patients in Carvedilol group were treated with Carvedilol in addition to routine treatment. Results Compared with the control group,the plasma levels of PRA,ATⅡ,ALD,ET were decreased significantly (P0.01),and the plasma level of NO was significantly increased (P0.01) in Carvedilol group(P0.01).There were significant difference in VP,LVEF,VP and dp/d_~tmax between two groups(P0.01).EV and EV/AV were increased and AV was decreased in Carvedilol group(P0.05).The clinical symptoms were improved obviously in Carvedilol group(P0.01). Conclusion Carvedilol not only suppresses the over-activated RAS,but also reverses the dysfunction of vascular endothelium and improves cardiac systole and diastole function in the long-term treatment of patients with congestive heart failure.
