Application value of color doppler ultrasound in diagnosing early deep venous thrombosis

Abstract

Objective To explore application value of color doppler ultrasound in diagnosis of early deep venous thrombosis. Methods396 cases examined by doppler ultrasonography from Jan. 1,2011 to Dec. 31,2012 in the First Hospital Affiliated to Fujian Medical University were observed on shape,location,echo and hemodynamic characteristics of deep venous thrombosis. Application subjects curve( ROC) was applied to evaluate the diagnostic effect. Results 36 cases of deep vein thrombosis were observed. The rate of ultrasound detection was 9. 09%. The acute thrombosis accounted for 58. 3%( 21 /36),the sub-acute thrombosis 22. 2%( 8 /36),and the chronic thrombosis 19. 4%( 7 /36). There were widened diameter,uneven clear venous wall,low echoes,no blood flow signals in acute venous thrombosis. The echoes were enhanced and diversified in the sub-acute venous thrombosed segments. The vascular walls were uneven and vascular filling defects were defined. The diameter of blood vessel lumen tended to be normal or thinner in the chronic venous thrombus segments. The echoes were moderate or high and collateral circulation could be detected surrounding the thrombosed segments. The area under the ROC curve( AUC) was 0. 969,standard error 0. 022,95% CI 0. 926 ~ 1. 000,sensitivity 94. 4%,and specificity 99. 4%. Conclusions The echo,luminal changes,hemodynamic and other features of deep venous thrombosis showed by doppler ultrasound should be combined to consideration,which was high sensitive and specific,and it is worthy of clinical application.
