The effect of ulinastatin on plasma endotoxin level in patients with severe acute pancreatitis and evaluation of therapeutic efficiency

Abstract

Objective To observe the effect of ulinastatin on the plasma endotoxin level in patients with severe acute pancreatitis.Methods 40 patients with severe acute pancreatitis were divided into two groups randomly. Patients in the control group accept common treatment and traditional Chinese medicine therapy, patients in the experimental group were injected with ulinastatin additionally. On the 1st, 4th, 7th and 10th day after hospitalization, serum amylase, plasma endotoxin were detecteded in each group. The APACHEII score system was used to evaluate the therapeutic efficiency of ulinastatin on treating severe acute pancreatitis.Results The plasma endotoxin level of patients with severe acute pancreatitis on the first day after hospitalization was significantly higher than that of healthy people(1.10±0.23 IU/L VS 0.20±0.50 IU/L, \%t\%=4 410, \%P\%0.001). The levels of serum amylase and plasma endotoxin in the experimental group decreased more obviously than those in the control group (\%t\%=2.947 and 3.226 respectively, \%P\%0.01), and the APACHEII score of the experimental group on the 7th day was lower than that of the control group (\%P\%0.05).Conclusion The endotoxemia existed in the early stage of severe acute pancreatitis. The effect of ulinastatin on treating severe acute pancreatitis was affirmed by decreasing the plasma endotoxin level and improving prognosis.
