Evaluation of clinical effects of treatment of III type dislocation of acromioclavicular articulation with clavicular hook plate and tension band

Abstract

Objective To evaluate the surgical effects of clavicular hook plates(CHP) and tension band in the treatment of Ⅲ type dislocation of acromioclavicular articulation.Method 72 patients with Ⅲ type dislocation of acromioclavicular articulation were treated with clavicular hook plate and tension band respectively. All patients were followed up for 8 to 48 months(24 months on average).Result There were no loosening and breakage of plates and screws in the cases fixed with CHP,but there were 16 cases of Kirschner wire loosening and 2 cases of wire breakage in the cases fixed with tension band.According to Karlsson's criterion for the function of shoulder joint,there were 27 cases of excellent,8 of good,1 of poor in CHP group and there were 21 cases of excellent,10 of good,5 of poor in tension band group.Conclusion The treatment of Ⅲ type dislocation of acromioclavicular articulation with CHP has the advantages of reliable fixation and easy operation.Early postoperative functional exercise could be executed by patients and the recovery of shoulder joint would be well done.CHP fixation is an ideal surgical method for treatment of Ⅲ type dislocation of acromioclavicular articulation.
