Community-based Chronic Disease Management

Chronic disease has become a primary threat to the health of nationwide population.With characteristics of long course and easy to break out repeatedly,chronic diseases are affected by bad living behaviors and thus need continuous,comprehensive medical service.Community is the main place for chronic disease prevention and control.On basis of community,to establish the 'community–hospital-patients' integration management and to provide the corresponding services according to different stages are the effective ways to deal with chronic disease threat.
