Community of Chronic Disease Prevention and Control

Objective:To understand the community of chronic disease prevention and treatment of the status quo and explore suitable for community chronic disease control strategies.Methods:The community at home and abroad as well as information on chronic diseases of the community of chronic disease control and prevention of literature analysis,research.Results:Chronic disease death rates were rising trend of prevalence of chronic diseases prevalent factors;community chronic disease prevention and management in place,can not meet the health needs of community residents.Conclusion:Effective organizational system is the community of chronic disease prevention and control work smoothly,continuing the favorable protection,the implementation of chronic disease prevention and control work should be community centers,community health service organizations as a platform to control risk factors as the goal,a community-oriented health needs,To carry out community health education and promotion as the core of the integrated control.
