Effect of insulin pump combined with continuous glucose monitoring system on blood glucose control of elderly GDM pregnant women and perinatal prognosis

Abstract

Objective: To explore the effect of insulin pump combined with continuous glucose monitoring system on blood glucose control of elderly pregnant women with gestational diabetes mellitus( GDM) and perinatal prognosis. Methods: A total of 110 elderly GDM pregnant women were selected and divided into experimental group( 57 cases) and control group( 53 cases) according to the therapeutic schedule; the patients in control group were treated with traditional multiple subcutaneous injection of insulin,while the patients in experimental group were treated with continuous subcutaneous pump of recombinant human rapid-effect insulin combined with continuous glucose monitoring system( CGMS); the effects of blood glucose control,insulin doses,the average recovering time of blood glucose,complications of pregnancy,fetal morbidities,and the incidence rates of postpartum hypoglycemia in the two groups were compared. Results: In the two groups,fasting blood glucose,postprandial 2- hour blood glucose,glycosylated hemoglobin( Hb A1c),and fluctuation amplitude of blood glucose after treatment decreased significantly,the changes of blood glucose levels after treatment in experimental group were statistically significantly better than those in control group( P0. 05). Compared with control group,insulin dose in experimental group was lower,the average recovering time of blood glucose was shorter,the incidence rate of hypoglycemia was lower,there were statistically significant differences between the two groups( P0. 05). After treatment,the incidence rates of hypertensive disorder complicating pregnancy( HDCP),polyhydramnios,and premature rupture of fetal membrane( PROM) in experimental group were statistically significantly lower than those in control group( P0. 05); neonatal birth weight in experimental group was higher than that in control group,while the incidence rates of neonatal asphyxia,premature birth,and stillbirth in experimental group were lower than those in control group,there were statistically significant differences between the two groups( P 0. 05). Conclusion: Insulin pump combined with CGMS can control blood glucose more rapidly and stably and reduce maternal and infantile complications and the incidence rate of postpartum hypoglycemia significantly,which is an effective and safe therapeutic schedule for elderly GDM pregnant women.
