Comparison of different disease management effect of hypertension in community of Shanghai

Abstract

Objective To compare the effects of detail disease management,social disease management and the current hypertension management model in community. Methods A one-year comprehensive community hypertension intervention was carried on in 430 people of hypertension. All these people were assigned to an intervention group with social disease management resources (172 people),a intervention group with detail management (111 people) and control group (147 people). Results The awareness of hypertension of intervention group with social disease management resources was increased,the salt intake was decreased from 4.6g to 4.0g per day,and the blood pressure was decreased by 10.3/2.6 mmHg. Some of the indicators were better than the other groups. Conclusions The community hypertension intervention with social disease management resources can improve the efficiency of community disease management,and can be a complement of the detail disease mangement model or the traditional three-tire management model.
