Comparison of General Anesthesia with Combined General-epidural Anesthesia

Abstract

Objective:To compare general anesthesia with combined general-epidural anesthesia in the aspects of patients’steady level during and after surgery and the dosage of anesthetics.Methods: 40 patients with thoracic,upper abdominal,lower abdominal surgery were randomly divided into simple general anesthesia group(G group,n=20)and combined general-epidural anesthesia group(GE group,n=20).GE group firstly underwent corresponding intervertebral puncture,up placed tube 3 cm,injected 2% lidocaine injection 3~5ml into epidural space.After determining block level,injected 0.5% ropivacaine 5~7ml into the chest and upper abdomen and 0.75% ropivacaine 7~9m into lower abdomen.Both groups utilized diazepam,propofol,fentanyl and cisatracurium besylate for induction and tracheal intubation.Micro-infusion pump was used to maintain general anesthesia through a continuous infusion of propofol and remifentanil,adjusted the anesthetics dosage according to the depth of anesthesia,and intermittent intravenous cisatracurium besylate.GE group could add ropivacaine according to the extending time of operation.Patients’blood pressure,electrocardiogram,pulse oxygen saturation were Monitored during operation.Results: The dosage of anesthetics in combined general-epidural anesthesia group was significantly less than that of general anesthesia group(P0.01),postoperative recovery early,extubation time shorten,and cardiovascular response was more stable than general anesthesia group.Conclusion: Combined general-epidural anesthesia can overcome one's weaknesses by acquiring others' strong points of each other,anesthesia process is stable,anesthetics dosage is significantly reduced.Postoperative analgesia can reduce many chest complications,improve and accelerate postoperative recovery.
