Investigation and analysis of perioperative prophylactic application of antibiotics in 596 patients with intestinal surgery

Abstract
Objective: To evaluate perioperative prophylactic application of antibiotics for patients with intestinal surgery and its clinical significance.Methods: 596 cases who were performed the intestinal surgery from January 2010 to December 2012 in our hospital(489 cases of innocuousness disease and 107 cases of malignant disease) were collected.The data of antibiotics clinical application,such as the selection of antibiotics species,medication timing,treatment,combination therapy,costs were analyzed retrospectively.Results: In 596 cases,584 cases with the stage I-III incision used the preventive antibiotics(97.99%),the cumulative drug use were 698 times,the average application was 1.20 times per case.In 584 patients,preventive medication included preoperative 1 d medication in 52 cases(8.90%),preoperative 2-24 hour medication in 64 cases(10.96%),preoperative 2 hour before surgery medication in 240 cases(41.10%),postoperative 24 hour ward medication(39.04%) in 228 cases.The average medication was 8.02 ± 3.53 days,a single medication in 387 cases(55.45%),the bigeminy medication in 289 cases(41.40%),triple therapy in 22 cases(3.15%).Preventive antibiotics included 11 major categories and 36 kinds,the top three antibiotics were cephalosporins of generation I,II and cephamycins respectively.There were intravenous administration of 584 cases(100%),combined oral administration of 64 cases(10.96%),topical administration of 12 cases(2.05%).The incidence of adverse reactions was 4.58%(32/698).Conclusion: It is necessary to enact the relative measures for controling antibiotics excess use in the perioperative patient with intestinal surgery.
