Abstract
Objective To explore the effect of remifentanil and ketorolac tromethamine in combination or alone on quality of resuscitation in patients undergoing general anesthesia. Methods 60 ASA Ⅰ~Ⅱ patients undergoing maxillofacial operation were randomly divided into four groups with 15 per group:group I remifentanil infusion was stopped at the end of operations, group Ⅱ remifentanil infusion was reduced to 0.05μg/(kg·min)at the end of operations and lasted to extubation, group Ⅲ 30mg ketorolac tromethamine was intravenously administered at 30min before completion of the operation and remifentanil infusion was stopped, group Ⅳ ketorolac tromethamine was used as group Ⅲ and remifentanil was used as group Ⅱ.The endotracheal extubation time, the time of goggle, cough and restlessness during resuscitation were recorded.BP and HR were monitored at different time points:at the end of operation,at 0,5 minutes and 10minutes after extubation, and the scores of verbal rating scale(VRS)at 5 and 10min after extubation were determined. Results Compared with group I,incidence rate of cough was low(P0.05),and the time of goggle and the endotracheal extubation time were long in groupⅡ and group Ⅳ.BP and HR were lower in group I than that in groupⅡ,Ⅲ and Ⅳ at 0 and 5minutes after extubation(P0.05 or P0.01),and were still low in group Ⅲ and group Ⅳ at 10minutes after extubation(P0.05 or P0.01).There were no significant differences in BP and HR at different time points in group Ⅳ(P0.05).The scores of verbal rating scale was lower in groupⅡ,Ⅲ and Ⅳ than that in group I at 5 minutes after extubation(P0.05)and was lower in group Ⅲ and Ⅳ than that in group I and Ⅱ at 10 minutes after extubation(P0.05 or P0.01).Conclusion Adminsitered remifentanil or ketorolac tromethamine alone can reduce adverse effects during recovery from general anesthesia, and the combined administration of remifentanil and ketorolac tromethamine makes extubation comfortable, painless and safe.
