Abstract
Our government has taken a new reform in community health service to meet the challenges in prevention of chronic deseases.However,prevention of chronic deseases is still a hard task because of many problems in community health service work. As the innovative method of health service, health management plays an important role in chronic disease prevention and community based health service.Health management contributes to adjusting community health service model,two-way referral system in prevention of chronic deseases, and monitoring the key patients with chronic deseases in community,therefore it plays an important role in prevention of chronic deseases in community.
