Investigation of operation mode of community health promoting work in integrated prevention and control for chronic disease

With the continuous development of the social economy,chronic diseases increasingly and seriously threat en the health state of residents.Currently,there are a lot of problems in the community health promoting work;this has brought great challenges to integrated prevention and control of chronic diseases.The innovation of community health promoting work plays an important role in the integrated prevention and control of chronic diseases.Along with the constant improvement of the mechanism of community health promoting work,it is particularly important to pro mote the prevention and treatment of chronic diseases through the influence and complement each other of health management and community service.Therefore,the community health management service mode for integrated preven tion and control of chronic diseases should be explored in the community health promoting work of our country,ac cording to the current situation of integrated prevention and control of chronic diseases,national circumstances and health service resources.
