Analysis on effect of community health files on hypertension management and intervention

[Objective]To study the effect of community cohort mode for hypertension management and intervention on the awareness rate,control rate and treatment rate of hypertension in patients,provide the basis for further hypertension control and management in community.[Methods]1 183 hypertension patients who were collected from 6 community health service centers in Liunan district and Liubei district of Liuzhou City were randomly divided into the intervention group and the control group.The intervention group was given the cohort hypertension intervention with a group mode,and the control group was given the conventional three-level management mode.After 6 months of intervention,the follow-up investigation was conducted again among the patients by the Hypertension patient follow-up service sheet.The awareness rate of knowledge about hypertension,control rate of hypertension,and changes of blood pressure value between two groups were compared.[Results]After intervention,the awareness rate of knowledge about hypertension,control rate and treatment rate of hypertension in the hypertension group were significantly higher than those in the control group(P0.05).[Conclusion]The community cohort mode for hypertension management and intervention can effectively manage the hypertension patients,reduce the risk of cardiovascular diseases and total mortality,as well as improve the quality of life in patients.
