Abstract

Objective To implement intervention measures on irrational prescriptions so as to promote clinical rational drug use and ensure the safety and effectiveness of patients' medication. Methods A total of 837 irrational prescriptions intervened by pharmacists from the Hospital of Ningde City Affiliated to Fujian Medical University between January 2012 and December 2013 were analyzed retrospectively. Results 869 cases of irrational drug use were examined, mainly including incorrect input of medical order, irrational usage and dosage of drug, improper solvent, unreasonable combined use, incompatibility and improper drug selection. After intervention by pharmacists, the proportion of adoption was 90.33%. Conclusion Intervention on irrational prescriptions beforehand can reduce irrational drug use effectively.Multiple intervention measures in advance aimed at irrational prescriptions should be improved and perfected constantly to improve rational drug use and ensure medication safety.
