Abstract
【Objective】To compare the efficacy and safety of intravenous diltiazem,amiodarone and cedilanid for the treatment of atrial fibrillation with rapid ventricular response in emergency department.【Methods】A total of 83 patients with rapid atrial fibrillation were randomized into 3 groups.Cedilanid group(n=24) were treated with intravenous 0.4mg cedilanid and repeated in 20min if heart rate(HR) 100bpm.Diltiazem group(n=31) were treated with intravenous 10mg diltiazem and repeated in 20min if HR100bpm.Amiodarone group(n=28) were treated with intravenous 150mg amiodarone and repeated in 20min if HR100bpm.【Results】The total efficacies were 75 %,93.5 % and 92.8 % in the cedilanid group,diltiazem group and amiodarone group,respectively.Mean time response to the bolus was 40.1±10.3,8.9±2.5 and 14.1±8.7 minutes,respectively.Two patients in diltiazem group and 2 patients in amiodarone group had asymptomatic hypotension.No patient developed worsening of congestive heart failure due to diltiazem.【Conclusion】Intravenous diltiazem or amiodarone is more effective than cedilanid in quickly decreasing of rapid ventricular rate in patients with atrial fibrillation.
