Abstract
Objective To demonstrate the diagnostic value of serum cystatin C and homocysteine in diabetic nephropathy patients, and the relationship between the serum cystatin C and homocysteine level and glomerular filtration rate. Methods A total of 47 diabetic patients were divided into two groups based on GFR detection by using isotope 99mTc-DTPA renal dynamic imaging:early stage diabetic nephropathy group(n=27,GFR≥60 mL/min) and end stage diabetic nephropathy group(n=20,GFR60 mL/min).Serum cystatin C,homocysteine and urine albumin excretion rate were detected in all patients. Results The CysC and UAER in the end-DN group were higher than those in the early-DM group. The correlation analysis showed that GFR was negatively correlated with CysC,Hcy and UAER(r=-0.584,P=0.000;r=-0.547,P=0.000;r=-0.507,P=0.000),The correlation analysis were also performed in chronic kidney disease at stageⅡandⅢ,and GFR was also negatively correlated with Cys C and Hcy(r=-0.617,P=0.000;r=-0.431,P=0.018).Conclusion The levels of serum cystatin C, homocysteine and urine albumin excretion were significant increased in the patients of end diabetic nephropathy, especially in the patients of chronic kidney disease at stagesⅡandⅢ.CysC and Hcy can be used as new good makers for early diabetic nephropathy diagnosis.
