The effect of community-based nursing intervention on self-management in patients with diabetes

Abstract
Objective To study the influence of community-based nursing intervention on the patients' disease-related knowledge,change of unhealthy behaviors,self-monitor and control of blood glucose in patients with diabetes.Method The six-month community-based nursing interventions included establishing patient records and providing guidance in diet,exercise,medication and mental health.Then,the 108 patients' disease-related knowledge and self-management behaviors were evaluated before and after intervention.Results The patients' self-management behaviors,blood glucose control and other index were significantly improved after nursing intervention.Conclusion The community-based nursing intervention is proved to be effective to promote the patients' recovery.
