Clinical observation of switching from premixed insulin to glargine in patients with type 2 diabetes uncontrolled by premixed insulin

Abstract

Objective To evaluate the efficacy and safety of insulin glargine plus oral antidiabetic drugs (OADs) in patients with type 2 diabetes inadequately controlled with premixed insulin plus OADs. Methods 150 patients with T2DM inadequately controlled by premixed insulin plus OADs were randomly divided into two groups.Patients in group A were administrated to insulin glargine combined with Repaglinide or Metformin,while in group B premixed insulin was not discontinued and the dosage was titrated. The treatment targets were fasting blood glucose(FBG) 6 mmol/L and postprandial blood glucose(PBG) 10 mmol/L. The changes of FBG,percentage of HbA1C 7%,dosage of insulin per day,incidence rate of hypoglycemia and change of weight after 12 weeks’treatment were observed. Results There was not significant difference in percentage of HbA1C 7% between group A and B(P 0.05). However,the fall of FBG level,dosage of insulin per day,incidence rate of hypoglycemia and gain in weight in Group A were all better than group B with significant statistic differences(P 0.05). Conclusion Switching from premixed insulin to glargine was associated with similar compliance rate in HbA1C but significant fall in level of FPG,the reduction of the dosage of insulin per day,the fewer incidence rate of hypoglycemia and less weight gain.
